New Clients: Please arrive 10 minutes early to complete a brief health questionnaire.

 

Payment: Payment due at time of service. Cash, check, credit card or Jenzen gift certificates. 

 
Cancellation Policy: Cancellations made with less than 24 hours notice will be charged 50% of the service price. If you cancel your appointment with less than four hours notice or fail to show up for your appointment you will be charged 100% of the booked service.

Screening Questions:

  1. ​Have you had a new or worsening cough?

  2. Have you had a fever?

  3. Have you had shortness of breath?

  4. Have you been in close contact with anyone with these symptoms or anyone who had been diagnosed with COVID-19 in the past 14 days?

  • Anyone who answers yes to any of the following questions on the day of your appointment will need to be rescheduled.
